If you completed Level 2 Education with ATOPS - complete this 7 page packet prior to attending your 1st class:

To submit your Therapy intake packet:
1. Complete and e-mail prior to your 1st class to admissions@atops.org
2. Scan and e-mail prior to your 1st class to admissions@atops.org
If you did NOT complete Education with ATOPS and are transferring from another agency complete the following:
The following needs be completed prior to attending your 1st Level II Therapy class.

All of the pages are required and must be submitted.
To complete your application on your smartphone or tablet you will need to download the free adobe reader app.

Per Colorado State Regulations, all pages must be received prior to your 1st class . If you fail to submit all pages we can not admit you into our program
